На основании результатов исследования разработана и апробирована системадиагностических и лечебно-профилактических мероприятий у больных с различными формамиуролитиаза, внедрение которой способствует снижению частоты рецидивов в 2,2 раза (р<0,05) исокращению для 1 пациента стоимости потерянного рабочего времени на 4% и сроковстационарного пребывания на 0,88 койко-дня, за счет персонифицированного подхода клечению, в том числе инвазивному, и метафилактике.UROLITHIASIS: CLINICAL AND BIOCHEMICAL ASPECTS OF THE PATHOGENESIS,DIAGNOSIS AND TREATMENTSuleymanov Suleyman Israfilovich(Russia)The research is devoted to the problem of diagnosis, choice of a reasonable method of treatmentand prevention of recurrences of stone formation in patients with various forms of urolithiasis.
Renaltubular acidosis causes metabolic disorders that are important in the pathogenesis of urolithiasis. Theimplementation of the pathogenetic mechanisms of stone formation occurs in conditions of high level oflithogenic substances, the presence of metabolic preconditions of the stone matrix formation and theformation of crystal aggregates with participation of activators and inhibitors of the process. The use ofroutine diagnostic algorithm (clinical blood and urine tests, ultrasound, excretory urography) and thepractice of treatment prescription without complex laboratory screening are associated with a highfrequency of recurrences of stone formation in patients with various forms of urolithiasis.
The use ofmodern biochemical tests for the indication of lithogenetic activators makes it possible to improve theaccuracy of the diagnosis of various clinical forms of urolithiasis and to personalize the tactics oftreatment. Based on the results of the research the system of diagnostic, therapeutic and preventivemeasures was developed and tested in patients with various forms of urolithiasis, the introduction ofwhich helps to reduce the recurrence frequency by 2.2 times (p<0.05) as well as to reduce the cost ofwasted working time by 4% for 1 patient and the duration of hospital stay by 0.88 days, due to apersonalized approach to treatment and metaphylaxis..
